RN Case Manager

We are seeking a dedicated and experienced RN Case Manager to manage an acute care caseload of medical patients. The ideal candidate will have a minimum of two years of relevant experience and a passion for providing high-quality patient care.

Shift Information

This position requires availability for day shifts with the possibility of weekends and holidays.

Responsibilities

• Manage a caseload of acute care medical patients.
• Coordinate patient care and ensure the delivery of high-quality services.
• Collaborate with a multidisciplinary team to develop care plans.
• Evaluate patient progress and make appropriate adjustments to care plans.
• Provide education and resources to patients and their families.

Qualifications

• Minimum of 2 years of experience in case management or a related field.
• Must hold relevant certifications, including:
  • Basic Life Support (BLS)
  • Advanced Cardiovascular Life Support (ACLS)
  • Pediatric Advanced Life Support (PALS)
  • Additional nursing certifications such as CPI, ENPC, RNFA, TNCC, AWHONN, NIHSS, STABLE, ASLS are preferred.
