Physical Therapist (PT) – Home Health

Job Overview

We are seeking a dedicated Physical Therapist to join our home health team. This position focuses on providing quality care to adult and geriatric patients within their homes. The ideal candidate will possess strong clinical skills and demonstrate a commitment to improving patient mobility and overall well-being.

Key Responsibilities

• Conduct initial assessments, develop, and implement individualized treatment plans based on patient needs.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and their families on exercise, mobility techniques, and strategies for improving quality of life.
• Collaborate with other healthcare professionals to provide comprehensive patient care.
• Document patient interactions and maintain accurate and timely records in accordance with agency policies.

Qualifications

• Minimum of 1 year of experience in physical therapy; first-time travelers accepted with a screening period of 6 months.
• Current BLS certification required.
• Georgia state license or compact physical therapy license acceptable (check member state eligibility).
• Experience with Home Care Home Base (HCHB) is preferred; completion of an online HCHB course required if no prior experience.
• Ability to travel between multiple counties for patient visits.

Work Environment

This role is primarily home health-based, with a patient-to-therapist ratio of 1:5. Flexibility is required for floating within the scope of practice as needed.

Requirements

• Availability for a maximum of 7 days of requested time off (RTO).
• Willingness to work during major holidays when applicable.
• Full compliance with onboarding requirements necessary to start on schedule.

Additional Information

The position involves daily travel, and mileage will be reimbursed at the IRS standard rate. Scrubs can be worn in any color, and the typical work schedule will consist of a five-day, eight-hour shift.
