Home Health Registered Nurse (RN)

Job Summary

We are seeking a dedicated Home Health Registered Nurse (RN) to provide quality care to adult patients in their homes. The ideal candidate will have a strong background in home care and home health experience.

Key Responsibilities

• Manage a caseload of approximately 30 patients, ensuring timely and effective care.
• Conduct start of care assessments, recertifications, and daily visits.
• Collaborate with a clinical manager or educator for support while in the field.
• Participate in weekend rotations as scheduled and contribute to holiday coverage.
• Utilize provided equipment, including a phone and tablet, to facilitate remote documentation and communication.

Qualifications

• Must possess an active RN license in the state of employment.
• Minimum of 2 years of experience in a home health setting and 1 year on a Medsurg floor in a hospital.
• Current Basic Life Support (BLS) certification from the American Heart Association (AHA).

Schedule and Work Environment

The position requires five 8-hour shifts per week, with typical hours from 8 AM to 5 PM, Monday through Friday, and rotating weekends. Candidates should be willing to participate in an orientation program consisting of two weeks of shadowing with a preceptor.
