Social Worker – Care Coordination

Job Summary

We are seeking an experienced Social Worker specializing in Care Coordination to provide essential support and assistance to patients, families, and healthcare teams. The ideal candidate will have proven experience in care coordination and be committed to improving patient outcomes through effective communication and resource management.

Key Responsibilities

• Facilitate care coordination among multidisciplinary teams to ensure comprehensive patient care.
• Conduct assessments and develop care plans tailored to individual patient needs.
• Provide counseling and support to patients and their families throughout the care process.
• Assist patients in navigating healthcare resources and accessing necessary services.
• Monitor patient progress and adjust care plans as needed to meet evolving healthcare needs.
• Collaborate with community resources and services to enhance the continuum of care.

Qualifications

• Current BLS certification.
• Master’s degree in Social Work (MSW) or Licensed Clinical Social Worker (LCSW) designation.
• Minimum of 2 years of experience in care coordination or a related field.
• Demonstrated ability to work effectively within a team environment.
• Strong organizational and communication skills.

Shift Information

This position is for an 8-hour day shift, with weekend coverage required every other week. A total of 40 hours per week is guaranteed.
