LTC/SNF LPN

Job Description

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Long-Term Care/Skilled Nursing Facility team. The ideal candidate will provide high-quality patient care, assist residents with daily activities, and work collaboratively with healthcare staff to ensure the well-being of all residents.

Key Responsibilities

• Deliver direct nursing care to residents in accordance with established protocols and patient care plans.
• Assist with daily living activities and provide emotional support to residents and their families.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor residents’ health and report any changes to the healthcare team.
• Maintain accurate and up-to-date medical records.
• Participate in training and professional development opportunities.

Shift Details

• Shift Length: 12 hours
• Guaranteed Hours: 36 hours per week
• Schedule: 6:00 AM to 6:30 PM
• Work every third weekend and every other holiday.

Qualifications

• Must possess a current and valid LPN license.
• Basic Life Support (BLS) Certification required.
• Minimum of 2 years of experience in a Long-Term Care or Skilled Nursing environment.
