Home Health Aide

We are seeking a qualified Home Health Aide to join our team. This position involves providing essential care to patients in their homes within a rural setting, primarily covering four counties. Candidates must have prior home health experience and familiarity with Home Care Home Base software.

Job Responsibilities

• Conduct OASIS assessments and manage start of care cases.
• Visit patients in their homes with an expected workload of 4 to 6 patient visits daily.
• Utilize a tablet to track time and document patient interactions accurately.
• Provide care and support to elderly patients dealing with common issues such as falls and fractures.

Schedule

This position typically operates on a Monday to Friday schedule from 8 AM to 5 PM.

Dress Code

Business casual attire or Hunter green scrubs with a white or black top is required.

Qualifications

• Home health experience is required.
• Experience with Home Care Home Base software is necessary.
• A valid license must be held at the time of submission.
