Licensed Practical Nurse (LPN)

Job Description

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Long-Term Care/SNF team. The ideal candidate will provide high-quality patient care and support to residents, ensuring their well-being and comfort.

Key Responsibilities

• Provide direct patient care and assist with daily living activities.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor patient health and report changes in condition to the nursing staff.
• Maintain accurate and timely medical records using EMR systems.
• Collaborate with other healthcare team members to ensure comprehensive patient care.
• Assist in maintaining a safe and clean environment for patients and staff.

Shift Details

• Shifts: Both 8-hour and 12-hour shifts available.
• Primary hours: Variable, typically from 3:00 PM to 11:00 PM, with one 12-hour night shift from 7:00 PM to 7:00 AM each week.
• Weekend coverage: Every other weekend required.
• Holiday coverage: Every other holiday required.

Requirements

• Minimum of 1 year of LPN experience in a Long-Term Care or Skilled Nursing Facility setting.
• Current BLS Certification.
• Strong communication and interpersonal skills.
• Ability to work collaboratively in a team-oriented environment.
• Flexibility to adapt to changing needs and schedules.

Additional Information

• Assignment Length: 12 weeks.
• Guaranteed hours: 40 hours per week.
• Overtime may be available as per operational needs.
