Position Overview

This role is designed for individuals who are committed to improving patient outcomes and enhancing the quality of life for residents in a skilled nursing setting. As part of our team, you will work Monday through Friday, enjoy a predictable schedule, and have weekends off.

Key Responsibilities

• Assess and implement patient care plans.
• Collaborate with interdisciplinary teams to optimize patient care and outcomes.
• Monitor and document patient progress and response to treatments.

Schedule
