Skilled Nursing Position

Job Description

We are seeking dedicated professionals to join our team at a skilled nursing facility that specializes in post-hospital, short-term rehabilitation, as well as long-term and respite care. Our mission is to provide high-quality medical and rehabilitation services to individuals recovering from illnesses or injuries.

Schedule

• 35 hours per week
• Monday to Friday
• Weekend rotation required

Responsibilities

• Deliver compassionate and patient-centered care to residents.
• Assist with daily activities and ensure residents’ comfort and safety.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Monitor and document residents’ progress and any changes in their conditions.
• Support residents’ rehabilitation goals through effective communication and engagement.

Qualifications

• Relevant certification and training in skilled nursing care.
• Strong interpersonal skills and a compassionate approach to patient care.
• Ability to work effectively in a team-oriented environment.
• Commitment to maintaining high standards of service delivery.
