Licensed Practical Nurse (LPN)

Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in a skilled nursing facility. The ideal candidate will have a strong commitment to providing high-quality patient care while working collaboratively with a multidisciplinary team.

Responsibilities

• Provide direct patient care, including administering medications and treatments as prescribed.
• Monitor and document patient conditions and progress based on care plans.
• Assist in the development and implementation of individualized care plans.
• Communicate effectively with patients, families, and healthcare team members.
• Participate in patient assessments, evaluations, and care coordination.
• Assist with activities of daily living (ADLs) and promote patient independence.

Schedule

This position requires working a full-time schedule of 40 hours per week, Monday through Friday, with weekend rotation required.

Qualifications

• Current and valid LPN license in the state of practice.
• Experience in a skilled nursing or similar healthcare setting is preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively within a team-oriented environment.
• Commitment to providing compassionate and patient-centered care.
