This position is located in a skilled nursing facility dedicated to providing post-hospital short-term rehabilitation, long-term care, and respite care. The facility accepts Medicare, Medicaid, and most private insurances, focusing on meeting the medical and rehabilitation needs of individuals recovering from illness or injury.

Key Responsibilities

• Provide compassionate care and support to residents and patients during their rehabilitation and recovery process.
• Assist residents with daily activities, ensuring they receive proper care and attention.
• Collaborate effectively with nursing and rehabilitation staff, maintaining clear communication and teamwork.
• Ensure all medical documentation is completed accurately and in a timely manner.
• Participate in weekend rotations as needed, ensuring coverage and care continuity for residents.

• Flexible to work on weekdays and weekends, including on a rotational basis.
• Experience in skilled nursing or rehabilitation settings is preferred.
