Physical Therapist (PT)

We are seeking a dedicated Physical Therapist to join our team. This position offers a dynamic work environment, serving patients across various settings.

Position Overview

The Physical Therapist will work approximately 36 hours per week, with shifts typically lasting 8 hours. The primary settings include Skilled Nursing Facilities (SNF) and Home Health services, with responsibilities shared approximately 50/50 between both settings. Flexibility is required, as occasional coverage in neighboring locations may be necessary.

Responsibilities

• Evaluate and treat patients in SNF and Home Health settings.
• Supervise two Physical Therapist Assistants (PTAs).
• Manage a caseload of approximately 6 patients per day in Home Health, aiming for about 28 patients per week while achieving high productivity standards.
• Utilize software including Net Health for SNF and HCHB for Home Health documentation and reporting.

Qualifications

• Minimum of 1 year of experience in a related field.
• Current Basic Life Support (BLS) certification is required.
• New graduates are welcome to apply.

Additional Information

• Schedule: 5 days per week with flexibility for coverage across various locations as required.
• A point of contact will be available onsite during the absence of the Therapy Supervisor.
• Travel to Hastings or Grand Island for SNF or Home Health coverage may be necessary.
