Licensed Practical Nurse (LPN)

Position Overview

We are seeking a dedicated and experienced Licensed Practical Nurse (LPN) for an inpatient rehabilitation unit. The ideal candidate will have a minimum of 1+ years of nursing experience in a skilled nursing or other 24/7 skilled care environment, with a preference for those who have experience in Med Surg or inpatient rehabilitation settings.

Key Responsibilities

• Deliver high-quality nursing care to adults requiring inpatient rehabilitation services.
• Work collaboratively with the healthcare team to maximize patient health, function, and quality of life.
• Assist with patient assessments, care planning, and implementation of rehabilitation programs.
• Monitor and document patient progress using electronic health records (EPIC).
• Participate in regular patient care team meetings and educational sessions.
• Provide support to Certified Nursing Assistants (C.N.As) and respond to patient call lights as necessary for safety.
• Adhere to mandatory attendance and tardy policies.

Qualifications

• Active LPN license in the state of practice.
• Minimum of 1 year of nursing experience in skilled nursing or a related environment.
• Experience in Med Surg or inpatient rehabilitation preferred.
• Basic Life Support (BLS) certification is required.
• Familiarity with high-acuity patient care and rehabilitation processes.

Work Schedule

This position will require 12-hour shifts from 6:30 AM to 7:00 PM, including every other weekend and holiday rotations. Regular attendance and punctuality are essential to maintain a smooth workflow.

• Patient Ratios: 1:7
• Equipment: Various mobility aids and monitoring devices.
