Licensed Practical Nurse (LPN)

We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our team. The ideal candidate will possess a strong foundation in nursing, with a focus on inpatient rehabilitation or medical-surgical experience.

Job Responsibilities

• Provide high-quality nursing care to patients in an inpatient rehabilitation setting.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Administer medications and treatments as prescribed.
• Monitor patient progress and report any changes in condition to the nursing staff.
• Assist patients with daily activities and promote their independence.
• Respond to patient needs and provide support during their recovery process.
• Ensure compliance with safety and health regulations.
• Participate in team meetings and contribute to a positive work environment.

Qualifications

• Minimum of 1 year of experience in skilled nursing or another 24/7 skilled care environment.
• Inpatient rehabilitation or medical-surgical experience is preferred.
• Current Basic Life Support (BLS) certification.
• Proficient in EPIC or willing to learn our electronic health record system.

Schedule and Additional Information

• Shift: 12-hour shifts from 6:30 PM to 7:00 AM.
• Weekend and holiday rotations are required.
• Mandatory adherence to the attendance policy.
• 30-minute breaks are mandatory; staff may take two 15-minute paid breaks during a 12-hour shift.

Unit Profile

• Number of Beds: 49
• Patient Ratios: 1 nurse to 7 patients
• Equipment includes mobility aids, vital monitoring devices, and EHR systems.
