LTC/SNF LPN

Job Description

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Long-Term Care/Skilled Nursing Facility (LTC/SNF) team. The ideal candidate will be responsible for providing high-quality patient care to residents in a supportive environment.

Key Responsibilities

• Deliver patient care in accordance with established protocols and standards.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient health and report any changes to the healthcare team.
• Assist with daily living activities, including bathing, grooming, and feeding.
• Communicate effectively with patients, families, and staff to ensure continuity of care.
• Participate in care planning and documentation using electronic medical records (EMR).

Shift Details

• Combination of 8-hour and 12-hour shifts available.
• Day shifts: 6:00 AM – 2:00 PM / 6:00 AM – 6:00 PM
• Evening shifts: 2:00 PM – 10:00 PM
• Every other weekend and holiday coverage required.

Qualifications

• Minimum of 2 years of LPN experience in a long-term care or skilled nursing environment.
• BLS Certification required.
• Strong communication and interpersonal skills.
• Ability to work collaboratively within a healthcare team.

Additional Information

• Length of assignment: 13 weeks.
• Weekly guaranteed hours: 36.
• Overtime opportunities available.
• Orientation will include building familiarization and training on equipment.
• Common patient diagnoses include LTC residents with a focus on memory care and dementia.
