Job Description

The Licensed Practical Nurse (LPN) is responsible for providing comprehensive care to patients in Skilled Nursing and Extended Care settings. This role encompasses a range of tasks including assessing, planning, implementing, and coordinating patient care.

Responsibilities

• Conduct patient assessments and develop care plans.
• Implement and coordinate patient care services.
• Monitor patient progress and adjust care plans as needed.
• Provide education and support to patients and their families.
• Collaborate with healthcare team members to ensure optimal patient outcomes.
• Adhere to safety and infection control protocols.
• Maintain accurate patient documentation and records.

Qualifications

• Current LPN license to practice in Colorado.
• Basic Life Support (BLS) certification required.
• Previous experience in Skilled Nursing or Extended Care is preferred.
• Ability to work flexible night shifts and take on-call responsibilities.
• Willingness to work overtime as needed.
