Case Manager

Job Summary

We are seeking an experienced Case Manager to join our team. The ideal candidate will have a strong background in acute care and be able to provide comprehensive case management services.

Responsibilities

• Manage patient care plans and coordinate with healthcare providers to ensure optimal patient outcomes.
• Utilize clinical skills to assess and evaluate patient needs and goals.
• Work collaboratively with interdisciplinary teams to facilitate effective communication and care delivery.
• Float between like units as necessary based on operational needs.

Requirements

• Minimum of 2 years of acute care experience required.
• First-time travelers are welcomed as a screening option with Clinical’s approval.
• Availability to work every other weekend (EOW).
• All requested time off (RTO) must be submitted at the time of application.

Charting and Parking

• Charting will be done using Epic.
• Free parking will be available on the first day, with designated parking details provided during orientation.
