Case Manager Registered Nurse (RN)

Job Description

We are seeking a dedicated and experienced Case Manager Registered Nurse (RN) to join our team. The ideal candidate will possess strong skills in patient care management and have experience with Epic.

Key Responsibilities

• Manage a diverse patient caseload and coordinates care plans.
• Collaborate with healthcare team members to ensure the highest quality of care.
• Conduct patient assessments and develop individualized care plans.
• Monitor patient progress and adjust care plans as necessary.
• Ensure compliance with healthcare regulations and best practices.

Schedule and Contract Details

• Start Date: ASAP
• Shifts Available: 4×10 or 5×8 days
• Contract Length: 13 or 26 weeks
• Shift Times: Varying start times within the range of 07:00 to 18:00
• Weekend Requirements: Every other weekend or 4 weekend days within a 4-week schedule
• Guaranteed Hours: 72 hours per 2 weeks
• Floating: Clinicians may float within their scope to meet facility needs

Qualifications

• Current Alaska RN license (required for auto offer).
• 1 year of acute care experience.
• Epic experience is mandatory.
• BLS certification is required; ACM certification is preferred.
• Ability to travel; local candidates must reside at least 50 miles from the facility.

Additional Information

• Parking is free.
• Scrubs will be provided.
• Only 1 time-off request per 13-week assignment (unless it is 5 days or less); no holiday time off is permitted for auto offers.
