Care Coordinator RN

Job Summary

We are seeking a dedicated and experienced Care Coordinator Registered Nurse (RN) to join our team. This role requires strong clinical skills and expertise in care coordination to effectively manage patient care processes.

Shift Details

• Shift Type: 8-hour Day Shifts
• Guaranteed Hours: 40 hours per week
• Every Other Weekend (EOW) required
• First to Float as needed

Key Responsibilities

• Manage and coordinate patient care plans in collaboration with healthcare providers.
• Conduct comprehensive assessments of patient needs and develop appropriate care strategies.
• Facilitate communication between patients, families, and healthcare teams.
• Monitor patient progress and outcomes, adjusting care plans as necessary.
• Educate patients and families on treatment plans and available resources.

Qualifications

• Minimum of 2 years of experience in care coordination or related field.
• Current California RN License is required.
• Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) from the American Heart Association (AHA).
• Experience with Care Coordination within the Dignity/Common Spirit network is mandatory.
