Skilled Nursing Position

We are seeking dedicated professionals to join our team at a skilled nursing facility that offers post-hospital, short-term rehabilitation as well as long-term and respite care. Our staff is committed to addressing the medical and rehabilitation needs of individuals recovering from illness or injury.

Position Overview

This role includes responsibilities focused on providing high-quality care and support to patients in a rehabilitative environment. The position requires participation in a rotating weekend schedule, fostering a collaborative team approach in delivering care.

Key Responsibilities

• Deliver compassionate care and support to residents in accordance with their care plans.
• Assist with rehabilitation activities and encourage patient participation.
• Monitor and document patient progress and changes in condition.
• Participate in team meetings to discuss resident care and updates.
• Engage in ongoing training and development to enhance skills and knowledge.

Qualifications

• Relevant experience in skilled nursing or rehabilitation settings preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively within a team and independently.
• Flexibility to work various shifts, including a weekend rotation.

Schedule

This position operates on a full schedule of 40 hours per week, Monday through Friday, with required weekend rotations included.
