Physical Therapist (PT) / Physical Therapist Assistant (PTA) / Speech-Language Pathologist (SLP)

Job Summary

We are seeking dedicated professionals in the fields of Physical Therapy and Speech-Language Pathology to provide high-quality rehabilitation services. The ideal candidates will play a crucial role in facilitating post-hospital rehabilitation as well as offering long-term and respite care to our patients.

Responsibilities

• Assess and develop individualized treatment plans based on patients’ needs.
• Implement therapeutic interventions to improve patients’ functionality and quality of life.
• Collaborate with multidisciplinary teams to ensure comprehensive patient care.
• Perform ongoing evaluations and adjust treatment plans accordingly.
• Provide education and support to patients and their families regarding rehabilitation and overall health.
• Participate in weekend rotation as scheduled.

Qualifications

• Must possess a valid license in the relevant field (PT, PTA, or SLP).
• Experience in a skilled nursing or rehabilitation setting is preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively within a team-oriented environment.
• Commitment to providing compassionate and patient-centered care.

Work Environment

This position is based in a skilled nursing facility offering advanced rehabilitation services, including memory care. The environment is supportive and focused on promoting the well-being and recovery of patients.

Schedule

Full-time positions are available, requiring a consistent schedule of 40 hours per week with Monday through Friday availability and participation in weekend rotations.
