Case Manager

Job Responsibilities:

• Provide comprehensive case management services in an acute care setting.
• Collaborate with interdisciplinary teams to develop and implement patient-centered care plans.
• Facilitate communication between patients, families, and healthcare providers.
• Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
• Assist patients with accessing resources and services to enhance their care experience.
• Document patient care and progress in the electronic health record.

Requirements:

• Minimum of 2 years of acute care experience required.
• First-time travelers accepted with Clinical’s approval.
• Availability to work every other weekend.
• Ability to float between similar units based on staffing needs.
• All requests for time off (RTO) must be included at the time of submittal.

Additional Information:

• Charting will be done using the Epic system.
• Free parking will be available on the first day, with assigned parking provided during orientation.
