Case Manager

Job Description

We are seeking a dedicated and experienced Case Manager to join our team. This position requires working every other weekend and involves managing patient care effectively within an acute care setting.

Responsibilities

• Provide comprehensive case management services for patients.
• Coordinate patient care plans and collaborate with multidisciplinary teams.
• Ensure appropriate resources and services are utilized for optimal patient outcomes.
• Document patient progress and interactions accurately in the charting system.
• Float between similar units as needed based on departmental requirements.

Qualifications

• Minimum of 2 years of acute care experience required.
• First-time travelers accepted with prior clinical approval.
• Must be willing to work weekends every other week.
• Experience with Epic charting system is preferred.
• Must be able to include all requests for time off at the time of submittal.
