Case Manager

Job Description

We are seeking a dedicated Case Manager with a passion for providing quality care to patients in an acute care setting. This position is suited for individuals who thrive in dynamic environments and are ready to contribute to our team on a flexible schedule.

Responsibilities

• Provide comprehensive case management services to ensure that patients receive coordinated care.
• Work collaboratively with healthcare professionals to develop and implement care plans.
• Monitor patient progress and adjust care plans as needed to meet evolving patient needs.
• Participate in interdisciplinary team meetings to discuss patient cases and promote best practices.
• Assist with discharge planning and follow-up care to enhance patient outcomes.
• Document all interactions and interventions accurately in the electronic health record system.

Qualifications

• Registered Nurse (RN) with a valid nursing license.
• Minimum of 2 years of acute care experience required.
• First-time travelers are accepted, pending clinical approval.
• Ability to work every other weekend as part of the schedule.
• Willingness to float between like units based on operational needs.
• Excellent communication and interpersonal skills.

• Charting System: Epic
• Parking: Free parking available on the first day, with designated assignment parking provided during orientation.
• Support for housing will be offered to travelers who require assistance in finding accommodation.
