Case Manager

Position Overview

Join a dynamic healthcare team as a Case Manager. This role requires a dedicated professional with experience in acute care to facilitate patient care and ensure that high-quality service is delivered.

Responsibilities

• Manage and coordinate patient care plans, ensuring that patients receive appropriate services.
• Collaborate with multidisciplinary teams to support patient needs.
• Assist in assessments and evaluations to determine patient requirements.
• Provide support and education to patients and families regarding care plans and available resources.
• Advocate for patients’ needs and preferences throughout their care journey.

Requirements

• Minimum of 2 years of acute care experience required.
• Strong communication and interpersonal skills.
• Ability to float between like units based on operational needs.
• Must be willing to work every other weekend.
• Experience with Epic charting system is preferred.

Schedule

The position is a day shift, with five 8-hour shifts per week (08:00 – 16:30). There is no required call. Available candidates should be prepared to work weekends every other week.
