Speech Therapist

We are seeking a qualified Speech-Language Pathologist (SLP) with a minimum of 1 year of experience working in a Skilled Nursing Facility (SNF). The ideal candidate will possess expertise in Rehab Optima (Net Health) and hold an active California license.

Responsibilities:

• Assess and evaluate patients’ speech and language needs.
• Develop and implement individualized treatment plans to address communication and swallowing disorders.
• Monitor patient progress and adjust treatment plans as necessary.
• Collaborate with a multidisciplinary team to ensure comprehensive patient care.
• Educate patients and their families on various communication strategies and techniques.
• Maintain accurate documentation and records of patient evaluations and treatments.

Qualifications:

• Minimum of 1 year of experience in a Skilled Nursing Facility (SNF) setting.
• Current and active California Speech-Language Pathology license.
• Familiarity with Rehab Optima (Net Health) software preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively as part of a healthcare team.
