Case Manager Registered Nurse (RN)

Job Description

We are seeking an experienced Case Manager Registered Nurse to join our healthcare team. The primary role will involve managing patient care across various units while ensuring optimal outcomes and resource utilization.

Key Responsibilities

• Conducting assessments and developing patient care plans.
• Coordinating with interdisciplinary teams to ensure comprehensive care delivery.
• Monitoring patient progress and modifying care plans as necessary.
• Facilitating communication between patients, families, and healthcare providers.
• Documenting all patient interactions and care details in the electronic health record.
• Educating patients and families about health management and resources.

Qualifications

• Current Registered Nurse (RN) license.
• Experience in Case Management is required.
• Strong communication and interpersonal skills.
• Ability to work collaboratively within a healthcare team.
• Familiarity with electronic health records, specifically Epic.

Schedule

This position involves a Day shift with 5×8 hour schedules. Weekend availability may vary based on need and patient census.

Work Environment

The position may require floating between various units within a short-term acute care facility. Scrubs are provided for designated areas, while business casual attire is expected for the Case Management role. Parking is available at no cost.

Patient Ratios

Expected patient-to-nurse ratios are 1:16, ensuring a manageable workload and effective patient care.
