Registered Nurse (RN) – Case Management/Discharge Planning

Job Overview

We are seeking an experienced Registered Nurse to join our Case Management and Discharge Planning team. The ideal candidate will have a strong background in acute inpatient medical case management and discharge planning, with the ability to quickly adapt to a busy hospital environment.

Key Responsibilities

• Manage the discharge planning process for patients across various inpatient units.
• Work collaboratively with a multidisciplinary team, including social workers, to ensure comprehensive patient care.
• Assess and address the complex needs of patients and their families.
• Consult with rehabilitation, long-term care, and hospice services as necessary for patient discharge.
• Maintain accurate and thorough documentation in the electronic healthcare record.
• Facilitate the placement of patients in appropriate post-acute care facilities.
• Navigate financial assistance opportunities for uninsured or underinsured patients.

Qualifications

• Minimum of 2 years of Acute Inpatient Medical Case Management experience, with at least 1 year spent in a large, busy hospital emergency department (500+ beds, Level 1/Academic).
• Recent experience in acute care discharge planning for a diverse range of medical diagnoses is required.
• Experience with telephonic utilization management/review, insurance company collaboration, and related areas is not applicable.
• Familiarity with EPIC charting is essential.
• Experience in a Level 1 facility within the last three years is preferred.
• BLS Certification is required.

Working Conditions

This position operates Monday through Friday from 8:00 AM to 4:30 PM, with no required weekend shifts. Holiday coverage will rotate as needed. The case load typically includes managing 15-20 patients per day.

Additional Information

• An understanding of and compliance with the flu vaccination requirement is essential, with only medical exemptions being considered.
• A fingerprint background check is mandatory.
• Orientation includes 2 weeks of department-specific training alongside the case management team.
