Case Manager

Job Description

We are seeking an experienced Case Manager to join our team. The ideal candidate will possess the necessary skills and experience to perform care coordination and discharge planning effectively.

Key Responsibilities

• Perform care coordination and discharge planning for patients.
• Collaborate with healthcare teams to ensure comprehensive patient care.
• Manage patient caseload to meet the needs of the department and organization.
• Participate in relevant departmental meetings and discussions.

Qualifications

• Must have at least 2 years of acute medical Care Manager experience.
• Valid California state license required. Pending licenses accepted, but must be licensed upon start date.
• First-time travelers are welcome, but must complete a screening process.

Other Requirements

• Must be able to work typically one full weekend every four weeks, depending on department needs.
• No restrictions on time off (RTO) exceeding 7 days.

Work Environment

• Type of Facility: Acute Care Facility.
• Dress Code: Business Attire or Scrubs.

Shift Details

• Day shift: 5×8 hours from 08:00 to 16:30.
• No floating expectation.
• No block schedules available.
