Registered Nurse (RN) – Case Management

Position Overview

This role is for an experienced Registered Nurse specializing in Case Management and Discharge Planning. The RN will work within a busy hospital setting to efficiently manage patient care and facilitate discharge processes for a diverse range of medical diagnoses.

Job Details

• Unit Type: Case Management
• Shift Length: 8 hours
• Guaranteed Hours: 40 hours per week
• Shifts: Monday to Friday, 8:00 AM – 4:30 PM
• Weekend Requirements: None
• Holiday Requirements: Rotated as needed

Responsibilities

• Manage a caseload of 15-20 patients per day across all inpatient units.
• Collaborate with a team of social workers, comprising 15 full-time and 17 part-time professionals.
• Facilitate the discharge planning process, consulting with relevant departments such as Respiratory Therapy and Pharmacy as needed.
• Assist with complex patient placements in collaboration with local facilities, ensuring criteria for financial assistance is met.
• Work in a fast-paced environment, adapting quickly to the needs of the unit and patients.

Qualifications

• Minimum of 2 years of Acute Inpatient Medical Case Management experience.
• At least 1 year of experience in a large, busy hospital with over 500 beds and a Level 1 trauma designation within the last 3 years.
• Recent experience in acute care discharge planning.
• Heavy experience in telephonic utilization management, insurance company processes, rehabilitation, long-term care, and hospice is not applicable for this position.
• Strong computer skills and proficiency in EPIC charting.

Requirements

• BLS Certification is required.
• Flu vaccine is mandatory, with exemptions only for documented medical reasons.
• Completion of fingerprinting required prior to starting.

Orientation and Training

The selected candidate will undergo a comprehensive orientation lasting 2 weeks, which will include both general facility orientation and unit-specific training. The RN will work alongside the Case Management team for support during this period.
