Pharmacist

Job Summary

We are seeking a dedicated Pharmacist to join our team. The successful candidate will work a day shift with overnight on-call responsibilities, following a 7 on/7 off schedule. This role requires a strong background in pharmacy practice and is essential in providing high-quality patient care.

Schedule

• Day Shift: 0730 – 1800
• Overnight On-Call: 1800 – 0730 for 6 or 7 nights, aligned with the pay period

Key Responsibilities

• Manage and dispense medications effectively while ensuring patient safety.
• Collaborate with healthcare professionals to optimize patient medication therapy.
• Provide counseling and education to patients regarding their medications.
• Utilize the Epic electronic health record system to document patient care activities.
• Assist with medication therapy management and monitor patient outcomes.

Qualifications

• Graduate of an accredited School of Pharmacy: Required
• Doctor of Pharmacy (PharmD) degree: Preferred
• Minimum of 1 year of related experience: Required
• Inpatient clinical pharmacy experience: Preferred

Licenses and Certifications

• Pharmacist license in the state of practice: Required
• Basic Life Support (BLS) certification: Required
• All necessary licensure and certifications to fulfill pharmacist duties at the state level: Required
