Case Manager

Job Description

We are seeking an experienced Case Manager to join our team. The ideal candidate will have a strong background in acute care hospital case management, with specific expertise in care coordination and discharge planning.

Key Responsibilities

• Perform comprehensive care coordination for patients during their hospital stay.
• Facilitate effective discharge planning to promote patient recovery and transition to post-hospital care.
• Collaborate with healthcare teams to ensure continuity of care and optimal patient outcomes.
• Maintain accurate and timely documentation in accordance with hospital policies.
• Participate in team meetings and contribute to quality improvement initiatives.

Qualifications

• Minimum of 2 years of acute care hospital case management experience is required.
• Strong understanding of care coordination and discharge planning processes.
• Must possess a valid state license to practice as a Case Manager.
• First-time travelers will be considered and screened for clinical readiness.
• Availability for weekend rotation as per departmental needs.
• No more than 7 days of requested time off (RTO) is permitted.
