Case Manager

Job Overview

We are seeking a skilled Case Manager to join our team. This role requires acute care experience and involves working every other weekend. The ideal candidate will demonstrate a strong commitment to patient care and possess the ability to float between like units as needed.

Key Responsibilities

• Provide case management services to ensure optimal patient care.
• Collaborate with healthcare teams to develop and implement individualized care plans.
• Monitor patient progress and adjust care plans as appropriate.
• Assist in coordinating resources and services for patients.
• Ensure compliance with applicable regulations and standards.

Qualifications

• Minimum of 2 years of acute care experience required.
• First-time travelers are welcome, pending clinical approval.
• Must be available to work every other weekend (EOW).
• Experience with Epic charting system preferred.

Work Schedule

This position is for day shifts with 5×8-hour schedules (08:00 – 16:30). There are no block schedules or on-call requirements.
