Physical Therapist (PT)

Job Summary

We are seeking a dedicated and experienced Physical Therapist (PT) to provide comprehensive physical therapy services to patients in their homes. The ideal candidate will have a minimum of 2 years of experience in an acute, home care, or rehabilitation setting and will be responsible for assessing, evaluating, and implementing therapeutic plans for clients.

Job Details

• Shift: 8:30 AM – 5:00 PM, typically one Saturday per month
• Shift Length: 8 hours
• Guaranteed Hours: 40 hours per week

Responsibilities

• Provide physical therapy services based on a physician’s written plan of care.
• Assess and evaluate patients’ rehabilitative and functional status.
• Direct physical therapy treatment and develop individualized care plans.
• Instruct patients, families, and caregivers in the use of therapeutic appliances.
• Monitor patients’ progress and report changes in condition to the physician.
• Prepare clinical and progress summaries based on patients’ progress towards therapeutic goals.
• Train and supervise home health aides involved in patients’ rehabilitation.
• Identify additional home health service needs and make referrals as necessary.
• Participate in ongoing staff competency assessments and peer consultation processes.
• Ensure compliance with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and other regulatory requirements.

Qualifications

• Minimum of 2 years of experience as a PT in acute, home care, or rehabilitation settings.
• Current Basic Life Support (BLS) certification.
• Good organizational, oral, and written communication skills.
• Ability to evaluate home environments and assess for muscle strength, mobility, gait, and range of motion.

Additional Information

Scrubs are not required; appropriate attire includes slacks (khakis are acceptable) and a polo or button-up shirt. Mileage will be reimbursed at the GSA rate with prior approval.
