Physical Therapist (PT)

Job Description

We are seeking a dedicated Physical Therapist to provide high-quality care in home health settings. The ideal candidate will possess strong organizational and communication skills and must be comfortable working in various home environments.

Key Responsibilities

• Deliver patient care in diverse settings including homes, assisted living facilities, senior apartments, and board and care homes.
• Manage a caseload of 3 to 5 patients daily, including Start of Care visits and evaluations.
• Collaborate with a multidisciplinary team to ensure continuity of care.
• Supervise up to 2 Physical Therapy Assistants (PTAs) or Certified Occupational Therapy Assistants (COTAs).
• Complete all required documentation and case management tasks within designated time frames.
• Adhere to safety and professional standards as outlined in the dress code and conduct policies.

Work Schedule

The position requires availability Monday through Friday from 8:30 AM to 5:00 PM. Participation in weekend and holiday shifts may be required, though flexibility will be considered when requests for time off are made.

Qualifications

• Valid California Physical Therapist license is required.
• Minimum of 2 years of experience in a healthcare setting; experience in community or home health is preferred.
• BLS certification is required.
• Experience with EMR systems, specifically HomeCare Home Base (HCHB), is preferred.

Orientation and Training

New hires will undergo a comprehensive 3-week orientation consisting of online learning modules and practical field training, with opportunities for mentorship and support.

Additional Information

This role requires a current driver’s license and proof of automobile insurance. Mileage reimbursement is available at the GSA rate with prior approval.
