LPN – Rehabilitation

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our rehabilitation unit. The ideal candidate will be committed to providing exceptional care to patients recovering from various injuries and illnesses.

Position Information:

• Department: Chronic 1/106
• Number of Openings: 1
• Shift: 6:00 AM – 6:00 PM
• Weekly Guaranteed Hours: 36
• Required Certifications: BLS
• Years of Experience: 2+

Unit Information:

• Number of Beds: 34
• Patient Ratios: 1:7
• EHR: Pinpoint
• Patient Population: 16 years and older

Responsibilities:

• Provide high-quality care to patients in a rehabilitation setting.
• Monitor patient progress and report any significant changes to the nursing team.
• Administer medications and treatments as prescribed by the healthcare team.
• Assist in implementing individualized rehabilitation plans for each patient.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Maintain accurate patient records and documentation in accordance with policies.

Qualifications:

• Required Education: Completion of an accredited LPN program.
• Preferred Experience: Previous experience in rehabilitation nursing.
• Strong communication and interpersonal skills.
• Ability to work effectively within a team environment.
• Compassionate and patient-centered approach to care.

First Day Instructions:

• Scrub Color: Eggplant for RN/LPN, Gray for CNA
• Time to Report: 6:45 AM
• Who to Ask for Upon Arrival: Charge Nurse
• Parking: Available in the parking lot (not marked visitor)
