Licensed Practical Nurse (LPN)

Position Overview

We are currently seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our rehabilitation team. The ideal candidate will provide exceptional care to our patients and help them in their recovery journey.

Job Responsibilities

• Deliver nursing care to patients in a rehabilitation setting.
• Assist with daily living activities and promote patient independence.
• Monitor patient health and report any changes to the healthcare team.
• Administer medications and treatments as prescribed.
• Provide education and support to patients and their families.
• Maintain patient documentation in accordance with facility protocols.

Qualifications

• Licensed Practical Nurse (LPN) certification.
• Current Basic Life Support (BLS) certification.
• Minimum of 2 years of nursing experience preferred.
• Experience in rehabilitation or similar environment is an asset.
• Ability to work collaboratively within a healthcare team.
• Strong communication and interpersonal skills.

Unit Information

• Number of Beds: 36
• Patient Ratios: 1:7; can go up to 1:8
• Electronic Health Record (EHR) System: Wellsky

Work Schedule

This position is for day shifts, with a guaranteed weekly hours of 36. Weekend shifts are required.

• Time to Report: 1830
• Who to Ask For Upon Arrival: Charge Nurse
• Parking: Available in the back of the facility.
