Registered Nurse Case Manager

Job Summary

The Registered Nurse Case Manager plays a pivotal role in coordinating patient care management while ensuring the effective utilization of resources and compliance with external review agencies. This position involves providing ongoing support and expertise through comprehensive patient assessments, care planning, implementation of plans, and the overall evaluation of individual patient needs.

Key Responsibilities

• Coordinating the management of care to enhance the quality of patient satisfaction and overall health outcomes.
• Promoting continuity of care and cost-effectiveness by integrating functions of case management, utilization review, and discharge planning.
• Serving as a patient advocate by addressing the holistic needs of patients and facilitating resource utilization.
• Collaborating with interdisciplinary teams to enhance patient care processes.
• Conducting thorough assessments and evaluations to inform care planning and implementation.
• Ensuring that patient preferences and needs are prioritized throughout the care management process.

Qualifications

• Current Registered Nurse (RN) license in California.
• Two years of recent RN case management experience, preferably in long-term acute care (LTAC) or short-term acute care hospital (STACH) settings.
• Experience with discharge planning across acute care settings and strong interdisciplinary team collaboration.
• Current Basic Life Support (BLS) certification through the American Heart Association.

Work Schedule

This role follows a Monday through Friday work schedule, from 8:00 AM to 4:30 PM.
