Case Manager

We are seeking a dedicated and experienced Case Manager to join our team. The ideal candidate must possess strong acute care experience and be prepared to contribute effectively in a fast-paced environment.

Key Responsibilities:

• Manage patient care by coordinating services and resources.
• Develop and implement individualized care plans based on assessments and ongoing evaluations.
• Collaborate with healthcare team members to ensure comprehensive care delivery.
• Maintain up-to-date patient records and documentation using Epic charting.
• Participate in case reviews and adjustments as needed to improve patient outcomes.

Requirements:

• Must have at least 2 years of acute care experience.
• First-time travelers are welcome, pending clinical approval.
• Availability to work every other weekend is required.
• Willingness to float between like units based on departmental needs.
• All requests for time off (RTO) should be submitted at the time of application.

• Day shifts with a schedule of 5×8 hours (08:00 – 16:30).
