Job Summary

We are seeking a compassionate and skilled Home Health Physical Therapist to provide high-quality therapy sessions to patients in their homes. This role involves traveling to patients’ residences and delivering essential therapeutic care tailored to individual needs.

Job Details

• Unit Type: PT (Physical Therapy)
• Shift Length: 8 or 10 hours
• Weekly Hours: 40 hours (Monday to Friday)
• Average Patient Visits: 5 visits per 8-hour day
• Charting System: Netsmart (formerly Allscripts)
• Travel Compensation: Mileage reimbursement at 0.70 cents per mile

Responsibilities

• Provide personalized physical therapy interventions to patients in their homes.
• Assess patient needs and develop tailored treatment plans.
• Document patient progress and update charts using the designated electronic health record system.
• Collaborate with healthcare teams to ensure comprehensive patient care.
• Ensure compliance with all necessary regulations and standards of practice.

Qualifications

• Current Physical Therapist license required in the state of Oregon.
• Minimum of 1 year of experience as a physical therapist.
• Basic Life Support (BLS) certification required.
• Self-starter with the ability to adapt quickly and work independently.

Housing can be challenging, especially for candidates with pets. Special housing arrangements may need to be coordinated prior to placement.
