LTC/SNF Registered Nurse (RN)
Job Summary
We are seeking a dedicated and compassionate LTC/SNF Registered Nurse (RN) to join our team. This role involves providing high-quality care to the elderly population in a long-term care and skilled nursing facility, focusing on both physical and mental health concerns.
Key Responsibilities
- Deliver direct patient care to residents, ensuring their comfort and well-being.
- Administer medications and monitor residents’ medication compliance.
- Conduct patient assessments to evaluate health needs and develop care plans.
- Collaborate with interdisciplinary teams to implement and adjust treatment plans as needed.
- Document patient progress and report any changes in condition to appropriate personnel.
- Participate in quality assurance and improvement initiatives.
- Provide education and support to residents and their families regarding care and health management.
Qualifications
- Valid RN license and BLS certification are required.
- Minimum of 1 year of nursing experience in a long-term care or skilled nursing facility.
- Preferred experience in geriatric psychiatric care or LTC psychiatric settings.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team-oriented environment.
Shift and Scheduling
This position requires 12-hour shifts, working day shifts with a commitment to every other weekend and holiday. The guarantee is for 36 hours per week for twelve-hour shifts.
Additional Information
Our facility supports a diverse population of elderly residents, including those with dementia and other behavioral health conditions. Comprehensive orientation and training will be provided, including an opportunity to shadow experienced staff members on-site.
