LTC/SNF Registered Nurse (RN)

We are seeking an experienced LTC/SNF Registered Nurse (RN) to join our team. The ideal candidate will have a passion for providing care to the geriatric population, particularly those with psychiatric needs.

Job Responsibilities

• Provide nursing care to residents in a long-term care skilled nursing facility.
• Administer medications and treatments as prescribed, ensuring accuracy and safety.
• Conduct assessments of residents’ health status and document changes in the electronic medical record.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Educate residents and families about health conditions and care plans.
• Participate in ongoing training and competency assessments.

Shift Details

• Shift: 12-hour days
• Every other weekend and holiday required

Experience and Qualifications

• Minimum of 1 year of nursing experience, preferably in geriatrics or psychiatric care.
• BLS Certification is required.
• Experience with electronic medical records, specifically PCC, is preferred.

Facility Overview

This is a long-term care facility specializing in geriatric psychiatric care. The facility has a capacity of 55 beds and provides specialized programs for patients with dementia and other behavioral issues.

The nursing team benefits from a supportive environment with a manageable nurse-to-resident ratio. Day shift nurses will have substantial charting support, allowing for a focus on patient care.
