Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. The successful candidate will provide quality care to patients in an inpatient rehabilitation setting, focusing on facilitating recovery and enhancing patient well-being.
Key Responsibilities
- Deliver patient-centered nursing care in collaboration with the healthcare team.
- Monitor and assess patients’ health status, recognizing and reporting changes in condition.
- Administer medications and perform treatments as prescribed by licensed healthcare providers.
- Assist in developing and implementing individualized care plans based on patient needs.
- Support patients in their rehabilitation process, motivating and educating them on their recovery journey.
- Maintain accurate and timely documentation of patient care activities and outcomes.
- Foster a positive and safe environment for patients and their families.
Qualifications
- Valid state licensure as a Practical Nurse (LPN).
- Minimum of 2 years of nursing experience preferred, particularly in a rehabilitation or acute care setting.
- Strong interpersonal and communication skills, with the ability to work collaboratively as part of a team.
- Commitment to high-quality patient care and rehabilitation outcomes.
- Basic Life Support (BLS) certification required.
Work Schedule
This position requires availability for daytime shifts. Flexibility to travel may be necessary based on patient care needs.
