Responsibilities

• Assess patients to determine their mental, physical, and spiritual needs.
• Collaborate with the multidisciplinary health care team to assess, plan, implement, and evaluate individual patient care plans.
• Provide clear explanations and instructions to patients and their responsible parties.
• Utilize clinical judgment to identify and report changes in patient conditions.
• Contribute to a positive workplace by managing conflicts through professional communication.
• Meet Core Measure responsibilities related to patient care.
• Participate in continuing education and professional development.
• Maintain complete and accurate medical records.
